A tall and spindly individual, Kao is often described as having pale brown skin and round eyes. Their hair is a pale purple and cut in a "jellyfish" style. They have full lips and a subtle adams apple. They typically wear light coloured clothes and various masks. Kao only wears Otogakure Shinobi uniforms during missions with their genin or when they will be working with/be around Shinobi of different villages (i.e. the chūnin exams). Usually has face paint that mirrors Orochimaru's markings.

Born into the Watanabe clan in the land of wind, Kao was born sickly and with incredibly low chakra reserves. At around six months old, their parents (who were very young) sought out Orochimaru, unsure of how to deal with Kao’s sicknesses. They gave the infant over in hopes that the sacrifice could provide some good for the greater ninja world.
They were quickly put into the Hashirama Cell experiments but showed absolutely no changes as the experiments continued. They were taken out of the project a few weeks before Orochimaru gave up on it. They grew up in the lab after that, going through various other experiments. Before Orochimaru’s defection they would volunteer to be a test subject or to just help out around the lab. It was around this time Oro learned the HCE were a success with Kao which influenced him taking the young child with him when he left.

"Both victims and survivors of the Hashirama Cell experiments, Kao has a sense of connection with Yamato, however they have a sense of superiority to Tenzō as they think they were the “favored one” thanks to gaining more abilities of Hashirama’s and being taken out of the initial program earlier. Tenzō probably doesn't even know who they are for a long time."
"One of their favorite people. Saw him once when they were in Kesshōgakura and almost instantly fell in love. They find any excuse possible to go to Kesshō to see and bother him, not caring if Tourou is working or not. Incredibly affectionate to him and shamelessly will flirt in public. Everyone in Otogakure has heard about him and they probably want to sew Kao’s mouth shut."
